In general, the shoulder is a fairly complex set of bones, muscles, and tendons.

The injury generally occurs to either one of the smaller tendons or muscles.

The cause of a shoulder rotator injury can be from a number of different reasons.

  • Repetitive stress.  Over time you repeat the same "poor" technique, or too much tension, whatever, but slight imperfections in your method (tennis, throwing, etc.) will eventually cause slight tears, and yep eventually, either a tendon or muscle gets inflamed.  Ouch...

  • Traumatic event.  You do something such as land hard in a sport, really over extend your shoulder, or something where that shoulder just can't handle the strain.  Some shoulder muscle or tendon goes nuts and again, ouch...

  • Poor strength.  You take time off from your activity and do nothing to strengthen the muscles in your shoulders, arms, back, etc. while you're inactive, you go back out there, and sure enough, you're shoulder isn't prepared for the activity, and here we go again, ouch...

  • Sleeping.  Yep, we can find a way to lay there for hours on end in an awkward position and our shoulder isn't real happy with that position.  Over time that lousy position can get painful.
